Fax cover sheet guide

A cover sheet is the first page of a fax. It tells the person at the machine who the fax is for, who sent it and how many pages to expect.

What to include

  • Recipient name and fax number.
  • Your name, fax number and phone number.
  • Date.
  • Subject or reference, like a case or patient account number (keep sensitive details off the cover).
  • Number of pages, including the cover.
  • A short message, and a confidentiality notice if you send private documents.

When you need one

Use a cover sheet when a fax goes to a shared machine, like a front desk, clerk's office or medical records department. It helps the right person get the fax and shows quickly if pages are missing.

Questions

Is a cover sheet required?

Not by law in most cases, but many offices expect one, and it reduces misdelivered faxes.

Should I put private details on the cover sheet?

No. The cover is often seen first by whoever picks up the fax. Keep sensitive details in the document itself.
